We are seeking an experienced professional to lead our discharge patient flow process. This exciting new role will support care coordination and patient experience across various facilities.

Job Overview:

The Discharge Patient Flow Concierge Lead will provide high-level support related to care coordination, improving communication, patient and carer experience, and delivering on patient flow outcomes. They will work collaboratively with clinical multidisciplinary teams to ensure efficient hospital operations and high-quality patient care.

Key Responsibilities:
  • Develop and implement strategies to improve the discharge patient flow process through collaboration with clinical teams
  • Provide expert guidance to enhance staff performance and develop innovative workplace solutions
  • Analyse quantitative and qualitative data to evaluate projects and prepare high-level reports
Requirements:
  • Relevant tertiary qualifications in a health-related field or relevant equivalent work experience
  • Demonstrated experience in continuous clinical practice improvement, project management, and/or change management
  • Leadership skills to empower stakeholders and develop innovative workplace solutions
