Overview

Join to apply for the Social Worker/ Discharge Planner role at Ramsay Health Care.

Fixed Term part time (20 hours per fortnight) position until March 2026. Preference for Friday availability.

Responsibilities
  • Coordinating the patient discharge planning process.
  • Ensuring patients have a smooth transition from the hospital to the community with appropriate care and services.
  • Plan complex discharges with a multidisciplinary team.
  • Provide clinical leadership to nursing, medical and allied health providers and coordinate the strategic and operational planning for discharge activities and associated services across the hospital.
Skills and Experience
  • Degree in Social Work or other relevant Allied Health qualification.
  • Eligibility for membership of the Australian Association of Social Workers (or equivalent Professional Association).
  • Extensive knowledge of community services and other resources relevant to this role.
  • Prior experience in a discharge planning role.
  • Knowledge of and ability to implement contemporary discharge planning practices.
  • Demonstrated ability to effectively and efficiently manage a caseload.
  • Excellent interpersonal and communication skills (written and verbal).
  • High standard of customer service and a team approach.
  • Commitment to professional development and maintenance of current evidence-based practice.
  • Flexible and positive approach to work.
  • Experience with My Aged Care and the residential aged care process, Life Time Care and Support, and NDIS will be highly regarded.

Requirements
  • Must provide a National Police Check conducted within the previous 12 months.
  • Ramsay may require a Working-With-Children check and proof of immunisation against infectious diseases.
