Job Title:

First Response Case Worker

The First Response Team provides crisis intervention, triage and case management services to individuals and families in need.

As a First Response Case Worker, you will work effectively with other team members to provide comprehensive support that reflects our organization's vision, mission and values.

Key Responsibilities:

- Answer calls and respond to walk-ins in a timely manner, providing initial assessment and referral as appropriate.

- Establish relationships with clients that encourage self-determination and participation.

- Provide case management support to individuals in accordance with our case management models and standards of practice.

- Develop individual case plans in collaboration with clients and include a range of interventions that address their needs and goals.

Requirements:
  • Tertiary qualification in Community Services, Social Work or related field.
  • Relevant experience in a social service environment including client assessment and case management.
  • Previous experience providing case management type support over the phone is preferred.
  • QLD Working with Children Check.
  • Driver's Licence.
  • First Aid Certificate.
  • A willingness to undergo a National Police Check.
