Community Mental Health Clinician
Work with our team to provide crisis assessments, interventions, counseling and psychoeducation while coordinating access to psychosocial supports and community services tailored to individual needs.
Key Responsibilities:- Provide high-quality mental health services to consumers in acute care settings.
- Monitor and respond to changes in consumer risk status ensuring timely clinically appropriate escalation and intervention.
- Foster strong collaboration with internal teams and external partners to ensure seamless coordinated care.
- Degree in Social Work, eligible for membership of the Australian Association of Social Workers (AASW).
- Experience in community mental health, confident in acute and triage settings, risk assessment and crisis intervention.
- Committed to holistic person-centred practice that includes families and carers as part of the care team.
