Dedicated Community Case Manager

Role Overview:
Our organization is seeking a dedicated Senior Case Manager to support the delivery of high-quality services to vulnerable populations. This position plays a crucial role in ensuring the continuity of care and enhancing client outcomes.
In this capacity, you will be responsible for reviewing client progress against established support plans, providing guidance and support to junior staff members, and advocating on behalf of clients with other service providers and government agencies. You will also contribute to knowledge-sharing initiatives and maintain expertise in program-related areas.
To succeed in this role, you will require qualifications in community work, social work, psychology, or behavioral sciences, as well as demonstrated experience in a leadership capacity within the homelessness sector. Strong organizational skills, time management abilities, and proficiency in Microsoft Office packages are essential. Additionally, you must possess the ability to assess risk, detect potential threats, and resolve issues related to challenging client behavior.
