ABOUT THE ROLES
Three Case Worker opportunities.
The Family Support Network (FSN) Case Worker provides a case management approach across all aspects of service delivery, including the common entry point, referrals, screening and assessment, and the provision of targeted supports within both Assessment and Coordination (A&C) and Intensive Case Management (ICM) streams.
Operating from a strengths-based, culturally secure, and relationship-focused approach, this role supports families to build capacity, strengthen parenting, and improve safety and wellbeing through responsive case management and direct intervention, including in-home support.
About FSN:
Wungening leads the Armadale Cannington Family Support Network. Family Support Networks (FSNs) are a partnership of community sector services and the Department of Communities (Communities), providing a common entry point to services and delivering targeted support to families facing challenging situations and those most likely to be involved with the child protection system.
Responsibilities Overview:

Deliver timely and effective Assessment and Coordination (A&C) and Intensive Case Management (ICM) in line with the WA FSN Operating Framework.

Work in partnership with the Coordinator FSN to support service delivery and promote practice excellence across the team and network.

Conduct referral screening and triage, supporting prioritisation of complex family circumstances, allocation decisions, and waitlist management.

Provide effective assessment, support coordination and care planning, managing both A&C and ICM caseloads, in person, in-home visits, and phone.

Collaborate with the FSN Team to ensure comprehensive risk assessment and safety planning.

Engage with FSN and external partners to ensure a coordinated and holistic response to family needs.

Support families in managing challenging situations by facilitating access to appropriate services and promoting parenting strategies, protective behaviours, and life skills development.

Contribute to service improvement by identifying practice gaps and supporting reflective supervision.

Maintain accurate, timely, and confidential records in the client management system (FUSION) and other systems, including detailed case notes, assessments, plans, and correspondence.

Ensure all practices align with Wungening's values, policies, procedures, and guidelines.
ESSENTIAL CRITERIA
-
Commitment to empowering Aboriginal families, children and young people and their right to participate in discussions and decisions relating to the supports they receive.

Experience in Early Intervention and Family Services or relevant child protection work with Aboriginal families, children, and young people.

Demonstrated experience in a complex case management and case planning.

Experience working with trauma-informed, restorative, or therapeutic care models.

Commitment to achieve compliance, policy implementation, risk, and quality assurance in the FSN service environment.

Demonstrated ability in risk assessment and management.
-
Excellent communication and interpersonal skills, with the ability to engage and collaborate across diverse stakeholders.

Problem-solving, conflict resolution, and decision-making abilities.

Sound computer skills, experience working with MS Office programs and data management systems
