About the Role

This part-time position supports the delivery of chronic disease management, helping patients navigate long-term health concerns through high-quality care planning, health assessments, and follow-up support.

You will collaborate with healthcare professionals to coordinate and deliver compassionate, person-centred care, supporting prevention, education, and patient empowerment.

This role requires an experienced Registered Nurse who is AHPRA registered, has experience in community health or general practice (3+ years), and is confident with CDM, care plans, and health assessments.
