Care Support Specialist

About the Role:

We are seeking a motivated and empathetic Care Support Specialist to work directly with clients, assessing their needs, providing tailored support services, and connecting them to relevant community resources.

Key Responsibilities:

  • Maintain a safe and supportive environment for individuals with disabilities, ensuring their daily care routines are met with dignity and respect.
  • Develop and implement personalized support plans to address client needs, promoting independence and well-being.
  • Accurately record client progress and adapt support plans as necessary, ensuring continuous improvement.
  • Refer clients to external agencies and community resources, fostering connections that enhance their quality of life.
  • Collaborate with community groups, welfare organizations, government agencies, and private businesses to raise awareness about our services.
  • Provide family support, education, and care to children and individuals with disabilities, addressing their unique needs.
  • Supervise offenders on probation or parole, where applicable, promoting accountability and rehabilitation.

Qualifications:

  • A Certificate in Community Services, Social Work, Youth Work, or a related field is preferred but not required.
  • Minimum 1 year's experience in relevant roles, demonstrating expertise and commitment.

Requirements:

  • Prior experience in client assessment, case management, and community engagement, showcasing your ability to connect with diverse populations.
  • Excellent interpersonal and communication skills, enabling you to build strong relationships with clients, colleagues, and community partners.
  • Strong knowledge of community services, welfare systems, and support programs, ensuring you can provide comprehensive assistance.
