We are seeking a skilled and experienced Registered Midwife to join our team as an Advanced Midwifery Navigator. This role is designed for individuals who possess advanced clinical expertise in midwifery, high-risk antenatal care, coordination, and interdisciplinary care management.
The successful candidate will have proven ability to build strong relationships and teams across the health landscape, with a strong commitment to delivering collaborative care that integrates the broader multidisciplinary healthcare team.
About the RoleThis role aims to provide safe and efficient care for high-risk women through coordination, education, empowerment, and self-management. Four key principles guide this role:
- Coordinating patient-centered care
- Creating partnerships
- Improving patient outcomes
- Facilitating system improvements
To be successful in this role, you will need:
- Vaccination against certain vaccine-preventable diseases
In addition, you will need to demonstrate:
- Advanced clinical expertise in midwifery, high-risk antenatal care, coordination, and interdisciplinary care management
- A proven ability to build strong relationships and teams across the health landscape
The successful candidate will be responsible for:
- Working collaboratively as part of a multidisciplinary team to provide high-quality, patient-centered care
- Coordinating and facilitating patient-centered care for high-risk women
- Educating and empowering individuals to manage their own care
- Improving patient outcomes through evidence-based practice
