Job Title: Advanced Midwifery Navigator

We are seeking a skilled and experienced Registered Midwife to join our team as an Advanced Midwifery Navigator. This role is designed for individuals who possess advanced clinical expertise in midwifery, high-risk antenatal care, coordination, and interdisciplinary care management.

The successful candidate will have proven ability to build strong relationships and teams across the health landscape, with a strong commitment to delivering collaborative care that integrates the broader multidisciplinary healthcare team.

About the Role

This role aims to provide safe and efficient care for high-risk women through coordination, education, empowerment, and self-management. Four key principles guide this role:

  • Coordinating patient-centered care
  • Creating partnerships
  • Improving patient outcomes
  • Facilitating system improvements
About You

To be successful in this role, you will need:

  • Vaccination against certain vaccine-preventable diseases

In addition, you will need to demonstrate:

  • Advanced clinical expertise in midwifery, high-risk antenatal care, coordination, and interdisciplinary care management
  • A proven ability to build strong relationships and teams across the health landscape
