As a Care Coordinator, you will play a pivotal role in ensuring the smooth operation of our support services. Your responsibilities will include liaising with participants, referral partners, and support staff to achieve participant goals in care plans.
You will be responsible for linking clients and support staff to meet participant needs, ensuring that all hours are filled within a reasonable timeframe. This requires effective communication and problem-solving skills, as well as the ability to prioritize tasks and manage multiple projects simultaneously.
Your Key Responsibilities:
- Participant Liaison: You will work closely with participants to understand their needs and develop personalized care plans that meet their goals and objectives.
- Support Staff Allocation: You will allocate support staff to meet participant needs, taking into account factors such as availability, skill level, and previous experiences.
- Inductions and Onboarding: You will be responsible for onboarding new and returning participants, providing them with a comprehensive induction and ongoing support to ensure they feel comfortable and confident in their roles.
- Relationship Building: You will develop and maintain effective relationships with participants and support staff, fostering a positive and supportive environment that encourages open communication and collaboration.
- Quality Assurance: You will work closely with the Care Manager to action customer feedback or complaints, identifying areas for improvement and implementing changes to enhance the quality of our support services.
