Job SummaryWe are seeking experienced, enthusiastic, and highly organised professionals to join our team as Support Coordinators. You will be the essential link between participants and the services they need, helping them navigate their NDIS plans to unlock their full potential.
- Build a connected life for people outside of funded support.
- Guide participants as they implement their NDIS plans, helping them understand options to make informed choices.
- Coordinating the implementation of these supports, ensuring capacity and funding management are optimised.
- Support participants collect key evidence to support their changing needs, and funding requests when required.
- Explore meaningful opportunities in recreation, employment, housing, and more to enrich their lives.
- Ensure mainstream services step up and fulfill their commitments.
Achieve all relevant KPIs, including billable hours. Qualifications include social work, disability, mental health, or a related field. Experience in NDIS support coordination is also required.
