**Job Overview**

We are seeking a skilled Healthcare Navigator to join our team. As a Healthcare Navigator, you will be responsible for delivering a telephone-based coaching program that supports customers in managing their health and wellbeing.

The role involves providing person-centered coaching and support to help customers self-manage their condition or risk factors and achieve their health goals. You will also be responsible for connecting customers to relevant programs or services, guiding them to build on their knowledge, skills and confidence to support self-management of their health goals, and managing individual caseloads.

**Key Responsibilities**

  • Assist customers in developing action plans to address opportunities for health improvement that are valuable to the customer
  • Assess health status and provide person-centered coaching and support to help customers self-manage their condition/risk factors and achieve their health goals
  • Connect customers to programs or services that are right for them via outbound and inbound channels
  • Guide customers to build on their knowledge, skills and confidence to support self-management of their health goals
  • Manage individual caseloads of customers scheduling and managing all coaching interventions in line with program guidelines

**Requirements**

  • A degree and Clinical qualifications in Dietetics. Eligible for membership of Dietitians Association of Australia
  • Open to both new graduates and experienced health professionals
  • General acute and/or community health nursing experience ideal however not essential
  • Friendly and confident telephone manner, with experience providing clinical care and advice over the phone (Desirable)
  • Experience in Health Coaching is highly desired however not essential
  • Proficient and confident in windows-based computer software and ability to work simultaneously and efficiently with telephone-based duties
  • Strong communication skills with a demonstrated ability to educate and motivate individuals
