Job Description:

The Client Intake Specialist will be responsible for assessing and referring clients to community-based services. This role requires a caring, warm nature and a passion for working with people in need of care.

Key Responsibilities:

  • Undertake telephone assessments and referrals
  • Screen and determine eligibility, and assess required needs with new or current service users, carers, family and other persons as appropriate
  • Provide information and assistance in regards to community-based services
  • Co-ordinate care plans for new and existing clients
  • Promote a high level of customer service to internal and external customers
  • Respond to telephone calls promptly and appropriately
  • Complete all required documentation for contact centre intake and assessment protocols

Required Skills and Qualifications:

  • Understanding of Community Support Scheme would be beneficial
  • Ability to use motivational interviewing techniques to gather screening and assessment data in order to determine service user eligibility for access to QCSS services
  • Relevant qualifications in community services and/or relevant experience in a health related field
  • Excellent computer skills
  • Demonstrated sound written and verbal communication
  • Ability to work collaboratively with all members of the Access Point team
