Job Title: Service Delivery Coordinator

Are you passionate about delivering exceptional customer experiences? Do you have a knack for creating innovative solutions to overcome day-to-day challenges?

As a Service Delivery Coordinator, your role will be to coordinate the delivery of services to clients and support staff in achieving identified program outcomes. You will work closely with customers to increase their engagement in planning and delivering their supports, promoting choice and providing exceptional experiences by ensuring wellbeing is at the heart of everything we do.

Key Responsibilities:
  • Meet or exceed Coordination of Support targets
  • Support a caseload of customers to increase their capacity to understand, monitor and manage supports of their choice
  • Act as the first point of escalation for clients, sharing knowledge, information and expertise with other team members and stakeholders

Requirements:
  • Passion, Respect & Integrity
  • Proven experience developing creative solutions to overcome day-to-day challenges
  • Relevant Tertiary qualifications and/or significant knowledge and experience in a similar role
  • Strong communication and listening skills
  • National Criminal History Check (Police Check) and Working with Children Check
  • Valid Driver Licence
