Our organization is dedicated to providing comprehensive support services for individuals experiencing homelessness in the Melbourne area. We are seeking an exceptional Case Manager to join our team.
This role involves conducting proactive outreach to identify people in need and linking them to essential services, including housing, healthcare, and social support. The goal is to provide tailored assistance to help stabilize their lives and promote greater stability and hope.
The ideal candidate will possess strong communication and interpersonal skills, as well as experience working with vulnerable populations. A key responsibility will be to work closely with community organizations, healthcare providers, and other stakeholders to ensure comprehensive support for our clients.
