Job Description

The HaRH program focuses on supporting individuals experiencing homelessness, at risk of homelessness or living in high-risk accommodation. Our multidisciplinary team provides targeted engagement strategies, undertakes holistic health and wellbeing assessments, facilitates referrals to relevant services, and enhances client journeys to better health outcomes.

Key Responsibilities

- Lead the development and implementation of outreach engagement strategies for target groups living in Brimbank, Melton and Hobsons Bay LGAs.
- Provide meaningful case planning, including identification and initial assessment of issues impacting clients' health, wellbeing and housing.
- Facilitate referral pathways for clients presenting complex needs, both internally and externally.
- Contribute to data collection, recording and reporting processes.
- Develop and maintain key stakeholder relationships to enhance service delivery.
