Older Adults Healthcare Specialist

We are seeking a skilled healthcare professional to provide high-quality, evidence-based services to older adults in the community.
The successful candidate will be responsible for delivering expert assessment, education, advice, and referrals to aid older adults in managing incontinence problems.
The role involves:
- Providing health assessments and cognitive screening for clients in receipt of a Home Care Package.
- Working collaboratively as part of a multidisciplinary team to support people with dementia and their carers.
The ideal candidate will have:
- A degree qualification in nursing and registration with AHPRA as a Registered Nurse.
- Demonstrated experience in providing clinical care to older adults.
- Excellent interpersonal skills and highly developed communication (written and verbal) skills.
- Strong organisational, planning, time management, and prioritisation skills.
