Care Coordinator Job Overview">The Integrated Team Care (ITC) program is a critical component of healthcare services for Aboriginal and Torres Strait Islander communities in Queensland. As a Care Coordinator, you will play a vital role in providing culturally appropriate and holistic primary health care to these communities.">

Key Responsibilities:

  • Conduct comprehensive health services that incorporate promotion, prevention, early intervention, health assessments, and treatment in accordance with medical protocols and established community guidelines.
  • Recall clients and facilitate referrals according to approved processes, including case coordination activities.
  • Provide crisis intervention, advocacy, and refer clients to relevant internal and external service providers.
  • Maintain accurate records in Communicare to reflect eligibility for services and ensure compliance with approved processes.
  • Manage ITC registration and processing for new and current clients.

Requirements:

  • Hold a Certificate IV or higher in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice) and possess at least three years of direct experience in delivering clinical health care services.
  • Demonstrate knowledge and understanding of Aboriginal and Torres Strait Islander societies and cultures, enabling effective communication with community members.
