Job Title: Aboriginal Health Practitioner, Primary Care

This is an exciting opportunity to work as a qualified and passionate individual providing flexible, holistic, and culturally sensitive health care services to Aboriginal and/or Torres Strait Islander people.

  • To be successful in this role, you will need to hold a Certificate IV in Aboriginal Primary Health Care (Practice) and be registered with the Australian Health Practitioner Regulation Agency (AHPRA).
  • You must also be eligible to drive in NSW and have the ability to travel as required.
About This Role:
  • Work collaboratively with nursing staff, Integrated Care Service, health professionals, Corrective Services NSW, Aboriginal Community Controlled Health Organisations and other relevant health and community groups to ensure seamless, timely and effective health care delivery.
  • Advocate for patients culturally and ensure appropriate referrals to specialty services for best patient outcomes during their health journey.
  • Identify patients of concern and escalate to a senior health professional or practitioner when necessary.

  • A strong understanding of cultural competency and its application in healthcare settings.
  • The ability to work independently and collaboratively within a multidisciplinary team environment.
  • Excellent communication and interpersonal skills.
