Job Title: Psychosocial Recovery Coach

We are seeking a compassionate and skilled individual to support participants in achieving their personal recovery goals.

The successful candidate will play a crucial role in assisting individuals with psychosocial disabilities to build capacity, develop resilience, and achieve greater independence.

You will work collaboratively with participants, their families, and support networks by connecting them with a range of services that align with their individual needs across multiple providers.

Your expertise and empathy will empower participants to navigate their recovery journey with confidence.

About the Role:

  • Develop and implement personalized recovery plans in collaboration with participants and their support networks.
  • Provide coaching and mentoring to participants, helping them to set and achieve personal recovery goals.
  • Support participants in implementing their NDIS plans, with a focus on fostering independence and recovery.
  • Assist participants in building skills and strategies to manage their mental health and well-being.
  • Coordinate and liaise with various stakeholders, including healthcare providers, support services, and family members.
  • Monitor and evaluate the progress of participants, adjusting plans and supports as needed.
  • Advocate for participants' rights and needs within the NDIS framework.
  • Maintain accurate and up-to-date documentation and reports on participant progress.
  • Stay informed about best practices in psychosocial recovery and contribute to continuous improvement initiatives.
  • Build and maintain knowledge of local services and organizations that customers can utilize.
  • Liaise with plan managers to establish appropriate claim categories and ensure accurate fund distribution.
  • Assess and manage risk in line with organizational policies and procedures.
