Life-Changing Support Worker

This role involves delivering culturally safe three-year flexible support packages to participants who need help sustaining tenancies or moving into stable housing.
Key Responsibilities:- Deliver case management and service coordination activities through assertive outreach to participants.
- Liaise with other agencies, community resources, and services to provide comprehensive support.
- Build strong relationships with participants experiencing chronic homelessness.
- Support participants to move and settle into their tenancies, linking them with local communities.
- Work long-term with a small caseload, focusing on individualised goal planning and regular outreach appointments.
- Relevant degree qualification in social work or community services.
- Experience in case management and navigating complex service systems.
- Passion for sustaining tenancies and ending homelessness.
- Ability to adapt to participants' needs, including effective collaboration and de-escalation techniques.
