Comprehensive Disability Care Specialist

We are seeking an experienced disability support nurse to provide quality clinical services to individuals with a range of functional abilities and complex needs.
Purpose of the Disability Support Nurse Role- To develop comprehensive health care plans for people with disabilities and provide nursing care to meet their individual needs.
- Coordinate clinical supports within resources, policies, and guidelines to ensure effective day-to-day administrative processes.
- Build relationships with local community partnerships, including families and carers, and other stakeholders to promote commitment to employment equity and diversity, workplace health and safety, risk management, and ethical practices.
- Developing comprehensive health care plans that take into account the unique needs and abilities of each individual.
- Providing high-quality nursing care to individuals with complex needs, including those requiring urinary catheter, PEG, and bowel care.
- Coordinating clinical supports to ensure seamless delivery of services and achieving desired outcomes.
- Building strong relationships with community partners to foster a culture of inclusivity and respect.
- Maintaining accurate records and providing regular feedback to service managers on service provision.
- Tertiary qualifications or equivalent, holding a current Registered Nurse (RN) registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Current First Aid Certificate
- Current NDIS and WWCC certificates
- Strong communication and interpersonal skills, with the ability to build rapport with individuals, families, and communities.
- Experience working with adults and/or children with complex disabilities and a willingness to learn about Indigenous cultures and perspectives.
