Comprehensive Care Specialist

We are seeking an experienced Comprehensive Care Specialist to ensure that our clients receive comprehensive and coordinated care in the comfort of their own homes.
Key Responsibilities:
- Conduct initial phone assessments and develop client-centred care plans
- Coordinate in-home services to support clients receiving care at home
- Build and maintain effective relationships with various stakeholders, including healthcare professionals and specialists
- Monitor patient progress and update care plans as needed
Requirements:
- 3-5 years' experience as a Registered Nurse in an acute hospital setting
- Recent experience in care coordination (highly desirable)
- Strong customer service orientation and relationship management skills
- Excellent clinical reasoning skills
