Care ManagementA skilled Registered Nurse or Licensed Practical Nurse is sought to join our team in providing comprehensive and coordinated care to patients. As a Care Management Nurse, they will collaborate with primary care clinicians and healthcare teams to develop and implement personalized care plans that meet the unique needs of each patient.
Responsibilities include:
- Developing, implementing, and monitoring personalized care plans for clients, ensuring all services align with their health and wellness goals.
- Serving as the primary point of contact for clients and their families, providing regular updates on care plans and addressing concerns.
- Collaborating with healthcare providers, social workers, etc., to ensure clients receive comprehensive and coordinated care.
Qualifications include:
- Graduation from an accredited school of nursing.
- Valid Montana Nursing License (RN or LPN) or current temporary license.
- Experience in caring for patients with chronic disease.
