**Wound Care Nurse Practitioner Opportunity**

Job Overview:

The ideal candidate will be responsible for delivering high-quality, patient-centered wound care services to patients residing in skilled nursing facilities. This is a comprehensive role that requires collaboration with the facility wound care team to assess and treat patients with wounds.

Responsibilities:
  • Rounding and managing the wound care of the population at assigned SNFs
  • Collaborating and rounding with wound care nursing staff to assess and treat patients
  • Providing timely documentation of all patient care/encounters
  • Availability for phone/secure messaging consultations with facility nursing staff
Requirements and Qualifications:
  • Valid PA/NP license in the state of practice
  • Active DEA license (Theoria Medical will assist with application)
  • General Post-Acute/SNF knowledge or experience preferred
  • Familiarity with EMR systems and charting practices

About The Role:

This is an in-person position requiring daily rounds Monday-Friday. Charting/documentation can be completed off-site. The provider must be able to move inside a facility frequently and briskly to see patients, meet with managers, and attend to patient needs.
