We welcome all candidates interested in making a difference in healthcare delivery by joining Theoria Medical.Shift StructureWound rounds with partner facility wound care teamIn-person rounding shifts start in the morning in accordance with facility wound round schedule with flexibility within reasonShifts are typically daily Monday-FridayCharting/documentation can be completed off-siteThis is an in-person positionEssential Functions & ResponsibilitiesProvider is responsible for rounding and managing the wound care of the population at the assigned SNFThe provider is responsible for collaborating and rounding with wound care nursing staff to assess and treat patients residing at SNFs with woundsProvider will provide high quality, patient-centered, comprehensive wound care services to patients residing in SNFsProvider is responsible for timely documentation of all patient care/encountersThe provider will be available for phone/secure messaging consultations with facility nursing staffProvider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasyPointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systemsPhysical RequirementsMust be able to move inside a facility frequently and briskly in order to see patients, meet with managers and facility staff, and attend to patient needsPunctual in-person attendance (typically between 7 a.m. and 1 p.m. is required for rounders)Continuous operation of app-based EMR/notes system either through a computer or on a mobile device (iPad, Phone, etc.)Ability to ascend and descend stairs in a facilitySome ability to position oneself to obtain records, tools, medical equipment and other items that may be in locations that are below the knee or above the shoulderEffectively communicate with patients, managers, and facility staffAbility to observe details in close range (within a few feet) in order to provide accurate and appropriate medical recommendations, treatment, and timely updates to managers regarding facility or patient concernsMust be able to lift or move at least 25 poundsRequirements and QualificationsCandidate must hold a valid PA/NP license in the state in which they will practiceCandidate must hold a valid NP/PA degree from an accredited institutionCandidate must have an active DEA license (If new graduate, Theoria will assist with your application)General Post-Acute/SNF knowledge or experience preferredCandidate must be familiar with EMR systems and charting practicesCompensation and BenefitswRVU based salary with monthly productivity bonusesPaid Time OffPaid holidays401k with employer matching and participationMedical, vision, and dental insuranceShort term and Long term disability insuranceMalpractice insurance covered by employerMedical licensure expenses reimbursed by employerOpportunities for training (i.e. wound care)All equipment and practice expenses coveredEmployee must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.
