AOD Navigator Role

We are seeking a skilled professional to fill the position of AOD Navigator. As an integral part of our team, you will play a crucial role in providing person-led, culturally appropriate, evidence-based care.

Key responsibilities include assessments, care planning, case management, and coordination with service providers. You will deliver support in person, by phone, and online, ensuring clients are linked to the right health, community, and support services.

You will also contribute to group programs, maintain accurate records, and participate in care coordination meetings. Regular travel within the region is required, as well as flexibility to work from designated offices.
