AOD Care Navigator

Job Description:

The AOD Hub offers an exciting opportunity to be part of a specialist, culturally responsive treatment and support service delivered in partnership with Armajun Aboriginal Health Service. The role involves providing person-led, culturally appropriate, evidence-based care across the New England region, focusing on rural and regional communities.

As an AOD Care Navigator, you will provide assessments, care planning, case management, and coordination with service providers. You will deliver support in person, by phone, and online, ensuring clients are linked to the right health, community, and support services.

You will also contribute to group programs, maintain accurate records, and participate in care coordination meetings. The role involves regular travel within the region and flexibility to work from Armajun offices in Inverell, Glen Innes, or Tenterfield.

  • Cultural competency and understanding of the needs of rural and regional communities
  • Ability to work in a team environment and build strong relationships with service providers
  • Strong communication and interpersonal skills
