This role presents an exceptional chance for a skilled Enrolled Nurse to join our Home Care team in the Central Coast.

The successful candidate will work as a Care Partner, responsible for assessing and coordinating care and support services for individuals receiving Commonwealth Home Support Services and Home Care Packages.

Key responsibilities include providing clinical care, support, education, and resources to consumers, empowering them to make informed decisions and have choice and control over their services and support.
