As the Care Partner you will be responsible for assessing and coordinating care and support services for participants assessing Commonwealth Home Support Services and Home Care Packages.
As well as providing clinical care you will also provide support, education and resources to consumers so they can make informed decisions and have choice and control over the services and support they receive and ensure the services delivered are safe, effective and meet consumers' needs.

Role & Responsibilities: Managing individual care and therapy plans within own professional scope of practice or via appropriate delegation, referral and agreed reporting framework.
Demonstrating the highest level of collaborative communication across primary, nursing and allied health professionals to support an integrated approach to care delivery.
Provide clinical care within your scope of practice as required based on your training and education, skills and experience.
Identify individual consumer's needs and implement a care plan and budget in consultation with the consumer, carer and relevant stakeholders (internal and external) Ensure care plans are developed based on the goals, strengths and identified needs in consultation with consumers Leading and participating in service development activities that may enhance the older persons wellbeing, quality of life and preference to remain living in their own home
About you? Registration as an Enrolled Nurse Sound knowledge and experience working with people who are ageing from an enablement, restorative and wellbeing perspective Understanding of multidisciplinary teamwork and the ability to multi-task.
Proven ability to conduct comprehensive assessments and develop goal orientated care plans Computer and technology skills to enable use of contemporary systems Skilled to engage and motivate consumers, families, staff and stakeholders.
