About the Role:

We are seeking a qualified and reliable Care Coordinator with nursing experience to join our team.

The successful candidate will be responsible for meeting with clients and their families, assessing their needs, and building strong connections. This includes conducting in-home client care assessments and preparing care plans to best meet client requirements.

Some nursing care and in-home care services may also be provided as needed. The role requires strong organisational, problem-solving, and time-management skills.
