Senior Care Coordinator Role

\

We are seeking a qualified and experienced clinical professional to help our clients live empowered and independent lives at home.

\

Our mission is to improve the quality of life for those we serve. Our approach focuses on making communities happier, and healthier.

  • Meeting with clients and their families and coordinating their needs within their funding model will be a key responsibility in this role.

    \

  • Conducting client care assessments and creating care plans to best meet client requirements will be a significant part of this position.

    \

  • Handling information requests, feedback, and general enquiries will also be required.

    \

  • Scheduling services for our clients by rostering care workers dependent on their qualifications and availability is another key task in this role.

    \

  • Ensuring all documentation is accurately and comprehensively completed in our databases and software programs is essential in this position.

    \

  • Performing some community nursing care as needed may also be a requirement in this role.

\

To be successful in this role, you will need:

\

  • A current AHPRA Registration is essential.

    \

  • Qualifications and experience in Nursing are highly valued.

    \

  • Highly developed verbal and written communication skills are necessary.

    \

  • Strong organisational, problem solving and time-management skills are crucial in this role.

    \

  • Proficient computing skills are expected.

    \

  • Current Australian National Police Check & Working with Children Check are mandatory.

    \

  • Current CPR certificate is required.

    \

  • Full Driver's license and your own reliable, insured vehicle are necessary.

    \

  • The right to work in Australia is essential.

\

Preferred Skills Include:

\

  • Sound understanding of the Aged Care and Disability government funding systems is desirable.

    \

  • Demonstrated experience supporting staff is valuable.

\
