Senior Nurse Coordinator Role

  • Meet with clients and their families to coordinate their needs within their funding model.
  • Conduct client care assessments and create comprehensive care plans to meet client requirements.
  • Handle information requests, feedback, and general inquiries.
  • Schedule services for clients by rostering care workers based on their qualifications and availability.
  • E nsure all documentation is accurately and comprehensively completed in our databases and software programs.
  • Perform community nursing care as needed.
Key Qualifications:
  • AHPRA registration.
  • Qualifications and experience in Nursing.
  • Excellent verbal and written communication skills.
  • Strong organizational, problem-solving, and time-management skills.
  • Proficient computing skills.
  • Australian National Police Check & Working with Children Check.
  • CPR certificate.
  • Full Driver's license and own reliable vehicle.
  • The right to work in Australia.
PREFERRED SKILLS:
  • Sound understanding of government funding systems for Aged Care and Disability.
  • Demonstrated experience supporting staff members.
