We are seeking a skilled and experienced Behaviour Support Practitioner to join our team in Geelong, Victoria.
About the Role:
This is an exciting opportunity for a qualified professional to deliver high-quality person and family centred NDIS support services that empower participants to achieve their life goals.
The successful candidate will work closely with participants of all ages to assess and support them through behavioural challenges, developing individual support plans and intervention strategies to promote positive outcomes.
Key Responsibilities:
- Work effectively with participants, families, support teams, and other stakeholders to develop and implement support plans
- Assess and support participants through behavioural challenges, developing individualised plans and strategies
- Compile reports and letters for NDIS participants, other health professionals, and NDIS reporting requirements
- Monitor and review participant outcomes in line with NDIS funding
Requirements:
To be successful in this role, you will require:
- A bachelor qualification in Social Work, Psychology, Occupational Therapy, Counselling or other relevant Allied Health field
- Current registration with AHPRA or relevant professional body
- Must be assessed as a core level practitioner as per the PBS Capability Framework
- Strong knowledge and understanding of the National Disability Insurance Scheme (NDIS) and disability industry
- A valid Australian Driver's Licence
- A cleared Victorian Working with Children Check (WWCC)
- A cleared National NDIS Worker Screen check
