
Aboriginal Community Support Specialist

We are seeking an experienced and dedicated professional to work as an Aboriginal Liaison Officer within our organization. This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as a deep understanding of the needs and concerns of Aboriginal patients and their families.
Key Responsibilities:- Provide culturally safe and person-centered support to Aboriginal patients and their families during hospital stays
- Advocate for patients and families to ensure their needs are met
- Engage with community leaders and organizations to strengthen relationships and improve outcomes
- Work closely with social workers and other healthcare professionals to achieve shared goals
- Support discharge planning and referrals, and assist with education and advocacy initiatives
- Minimum Certificate III qualification in Aboriginal Primary Health Care or a related field
- Evidence of Aboriginal and/or Torres Strait Islander identity
- Ability to assess patient needs and advocate on their behalf
- Effective interpersonal and communication skills
- Knowledge of community-based support services and programs
- Competence in the use of databases and electronic documentation systems
