Our organisation is committed to making a positive impact in the lives of Aboriginal patients and families. As an Aboriginal Liaison Officer, you will play a vital role in supporting these individuals during their hospital journey.

Job Description
  • You will provide culturally safe, person-centred support to Aboriginal patients, families, and carers while they receive care at the hospital.
  • Your work will involve advocating for patients and families to ensure their needs are met during their hospital stay.
  • You will engage with the community to strengthen the Aboriginal Liaison Officer process and outcomes.
Key Responsibilities
  1. Provide culturally safe support to Aboriginal patients and families.
  2. Advocate for patients and families to ensure their needs are met.
  3. Engage with the community to strengthen the Aboriginal Liaison Officer process.
  4. Work closely with social workers to achieve shared goals for patients and their families.
  5. Build relationships between the hospital, patients, and the local Aboriginal community.
  6. Support discharge planning and referrals through programs like the Aboriginal Chronic Disease Management Program.
Requirements

We are looking for candidates who hold or aspire to hold a minimum Certificate III qualification in Aboriginal Primary Health Care or a related health qualification. Candidates must also demonstrate evidence of diversity claim if applicable: Aboriginal and/or Torres Strait Islander.

Selection Criteria
  1. Ability to assess the needs of Aboriginal patients and advocate on their behalf.
  2. Ability to establish effective working relationships with health professionals.
  3. Knowledge of community-based support services and programs.
  4. Effective interpersonal skills.
  5. Understanding of patient complaints and privacy processes.
  6. Competence in database use and computer systems.
  7. Demonstrated skill in analysis and problem-solving.
