Care Planning Coordinator Role

We are currently seeking a committed Care Planning Coordinator to join our Clinical Leadership team at our Kellyville home.

The successful candidate will be responsible for the overall clinical assessment program, supporting RNs to ensure resident assessments are completed, and informing the development of care plans tailored to residents' individual assessed care needs.

You will work closely with the Care Manager to establish a Care Plan review program, ensuring care plans and assessments remain current and relevant.

Key Responsibilities:

  • Establishing a Care Plan review program to ensure care plans and assessments remain current and relevant
  • Participating in the development of training and education plans to ensure employees are provided with the required support and education
  • Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy, and antimicrobial stewardship, and reporting and consent obligations
  • Assessment and care planning for new admissions and existing residents, in consultation with internal and external stakeholders, Care Team, health professionals, and Admissions Coordinator
  • Monitoring the delivery of care is consistent with the care plan and goals, including dignity of risk choices, and taking action as required if gaps or inconsistencies are identified
  • Reviewing care incidents and care change patterns for residents, and collaborating with the Clinical Care Team to determine if further assessment and care planning modifications are needed
  • Maintaining accurate client records, case notes, and other data as directed and required

Requirements:

  • Registered Nurse with at least 2 years clinical experience; aged care management experience and familiarity with the Quality Standards for Aged Care
  • A high level of knowledge and understanding of the Care Planning, Assessment, and Documentation in Residential Aged Care
  • Excellent and demonstrated verbal, written, and interpersonal communication skills
  • Knowledge and ability to use computers and technology efficiently
  • Current Police Check and appropriate Working Rights
  • Up-to-date vaccinations (Influenza, COVID-19)
  • Experience in a similar position in Aged Care - desirable
  • Management qualifications – highly regarded

Salary Estimate: AU$85,000 - AU$105,000 per annum, depending on experience
