The Opportunity

We are seeking a highly skilled Care Planning Coordinator to join our Clinical Leadership team at our Kellyville facility. This role offers an exciting opportunity to make a meaningful impact on the lives of our residents while working collaboratively with a talented team of professionals.

About the Role

As a Care Planning Coordinator, you will be responsible for the overall clinical assessment program, supporting Registered Nurses to ensure resident assessments are completed and informing the development of tailored care plans. You will also participate in clinical governance activities, including planning and implementing clinical priorities. Our team is dedicated to providing exceptional care, and we are committed to supporting your growth and development within the role.

Key Responsibilities:
  • Establishing a Care Plan Review Program: Work closely with the Care Manager to ensure care plans and assessments remain current and relevant, reviews are initiated when required, and residents' assessed care needs are well-documented.
  • Developing Training and Education Plans: Participate in the development of training and education plans to ensure employees are provided with the necessary support and education to competently complete assessments, care plans, and documentation based on accurate and comprehensive assessments, consultations, observations, and collaborations.
  • Assessment and Care Planning: Assess and develop personalized care plans for new admissions and existing residents in consultation with internal and external stakeholders, the Care Team, health professionals, and Admissions Coordinators.
  • Maintaining Accurate Records: Maintain accurate client records, case notes, and other data as directed and required.
Required Skills and Qualifications:
  • Registered Nurse with 2+ Years of Clinical Experience: Possess a high level of knowledge and understanding of care planning, assessment, and documentation in residential aged care.
  • Excellent Communication Skills: Demonstrate excellent verbal, written, and interpersonal communication skills.
  • Computer Literacy: Have the ability to use computers and technology efficiently.
