Role Overview
We are seeking a skilled Care Planning Coordinator to join our Clinical Leadership team at our Kellyville location. This is an exciting opportunity for a registered nurse with experience in aged care management to contribute to the development of high-quality care plans and assessments.
Key Responsibilities
- Establish Care Plan Review Program: Work closely with the Care Manager to ensure care plans and assessments remain current and relevant, reviews are initiated when required, and resident assessed care needs are well documented.
- Develop Training and Education Plans: Participate in the development of training and education plans to ensure employees have the necessary support and education to complete assessments, care plans, and documentation accurately and comprehensively.
- Regular Analysis and Reporting: Responsible for the completion of regular analysis of psychotropic, antipsychotic, polypharmacy, and antimicrobial stewardship, and reporting on consent obligations.
- Assessment and Care Planning: Assess and develop personalized care plans for new admissions and existing residents, in consultation with internal and external stakeholders, Care Team, health professionals, and Admissions Coordinator.
- Monitor Care Delivery: Monitor the delivery of care to ensure consistency with care plans and goals, including dignity of risk choices, and take action as required if gaps or inconsistencies are identified.
- Review Care Incidents: Review care incidents and care change patterns for residents, collaborate with the Clinical Care Team to determine if further assessment and care planning modifications are needed, initiate care plan reviews, and re-assessments.
- Maintain Accurate Records: Maintain accurate client records, case notes, and other data as directed and required.
Requirements and Qualifications
- Registered Nurse with 2+ Years of Experience: Aged care management experience and familiarity with Quality Standards for Aged Care are essential.
- Knowledge of Care Planning and Assessment: A high level of knowledge and understanding of care planning, assessment, and documentation in residential aged care is required.
- Excellent Communication Skills: Excellent verbal, written, and interpersonal communication skills are essential for this role.
- Technical Skills: Knowledge and ability to use computers and technology efficiently are required.
- Certifications and Requirements: Current Police Check, appropriate Working Rights, up-to-date vaccinations (Influenza, COVID-19), and experience in a similar position in Aged Care are desirable.
