Job Description

As a Healthcare Coordinator in our Clinical Leadership team at Moran Home at Kellyville, you will be responsible for the overall clinical assessment program, ensuring resident assessments are completed and inform the development of care plans tailored to residents' individual assessed care needs.

You will work closely with the Care Manager to establish a Care Plan review program, ensuring care plans and assessments remain current and relevant. You will also participate in the development of training and education plans to ensure employees are provided with the required support and education to competently complete assessments, care plans, and documentation.

The ideal candidate will have a strong understanding of care planning, assessment, and documentation in residential aged care, as well as excellent communication and interpersonal skills. Experience in a similar position in aged care is desirable, but not necessary.

Key Responsibilities:
  • Establish and maintain a Care Plan review program to ensure care plans and assessments remain current and relevant.
  • Participate in the development of training and education plans to ensure employees are equipped to competently complete assessments, care plans, and documentation.
  • Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy, and antimicrobial stewardship and reporting and consent obligations.
  • Assess and develop care plans for new admissions and existing residents in consultation with internal and external stakeholders, Care Team, health professionals, and Admissions Coordinator.
  • Monitor the delivery of care is consistent with the care plan and goals, including dignity of risk choices, and take action as required if gaps or inconsistencies are identified.
Required Skills and Qualifications:
  • Registered Nurse with at least 2 years of clinical experience; aged care management experience and familiarity with the Quality Standards for Aged Care.
  • A high level of knowledge and understanding of care planning, assessment, and documentation in residential aged care.
  • Excellent verbal, written, and interpersonal communication skills.
  • Knowledge and ability to use computers and technology efficiently.
  • Current Police Check and appropriate Working Rights.
  • Up-to-date vaccinations (Influenza, COVID-19).
