Job Title: Team Leader HACDSP

This is an exciting opportunity to take on a leadership role and contribute to the success of our organisation.

As a Team Leader, you will be responsible for leading a multidisciplinary team of staff across multiple locations. Your primary focus will be on ensuring that the Dementia Support Australia (DSA) service meets the needs of individual clients and delivers on its key performance indicators.

Key Responsibilities:
  • Leadership and Team Management: Provide leadership to your team, focusing on empowerment, mentorship, and support to ensure their wellbeing and safety.
  • Client Care: Deliver high-quality client care, prioritising their needs and providing timely allocation of referrals to the appropriate staff member.
  • Reporting and Analysis: Analyse operational performances, identify areas for improvement, and provide regular reporting to the Regional Manager.

We are looking for a highly skilled and experienced professional who can effectively manage multiple tasks and projects simultaneously.

The ideal candidate will have a strong understanding of psycho-social approaches to case management and be able to communicate effectively with stakeholders at all levels.

Requirements:
  • Tertiary Qualifications: Nursing/Allied Health or health-related degree.
  • Experience: Demonstrated experience in supporting older people and carers living with dementia.
  • Leadership Skills: Ability to lead and manage a team of professionals.
  • Communication Skills: Strong communication and interpersonal skills.
  • Organisational Skills: Ability to work on multiple tasks and projects simultaneously.
