The successful candidate will be responsible for fostering and cultivating long-term and sustainable relationships with participants, their family members, support workers, and other stakeholders. This involves creating a clear and consistent point of contact for all parties involved.
As a central point of contact, the Relationship Manager will assess participant needs, develop Hearth's support program, and maintain relationships with support coordinators, house supervisors, and other relevant external stakeholders. They will also lead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals.
Responsibilities include reporting on client progress, preparing submissions for NDIS plan reviews as required, coordinating staff rosters, and ensuring compliance with policies and procedures. Additionally, the successful candidate will have a proven client-centred approach, excellent time management and organisational skills, and high levels of resilience.
- Key Responsibilities:

- Essential Skills and Qualifications:
- Proven experience working in the disability sector, preferably in a supervisory or managerial role
- Professional level of administrative and business communication skills, including case noting and report writing
- Excellent time management and organisational skills
- Proven client-centred approach with the ability to advocate for Participants and an understanding of complex disability and trauma
- Strong interpersonal skills and high levels of resilience
