Job Description

As a key member of our team, you will play a vital role in fostering and cultivating long-term relationships with participants, support workers, and external stakeholders. Your primary responsibility will be to create and maintain a clear point of contact for all stakeholders, ensuring seamless communication and collaboration.

Key responsibilities include:

  • Developing and maintaining relationships between participants and their team of support workers
  • Assessing participant needs and developing personalized support programs
  • Maintaining relationships with support coordinators, house supervisors, and other relevant external stakeholders
  • Leading the support worker teams to provide responsive, person-centered care that enables participants to progress toward their goals

Additional responsibilities include:

  • Reporting on the progress of clients and preparing submissions for NDIS plan reviews
  • Coordinating a weekly staff roster for each participant and resolving any rostering issues
  • Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan
Requirements

We are seeking a highly skilled and experienced professional who possesses the following qualifications:

  • Proven experience working in the disability sector, preferably in a supervisory or managerial role
  • A high level of administrative and business communication skills, including case noting and report writing
  • Excellent time management and organizational skills
  • Proven client-centered approach with the ability to advocate for participants and an understanding of complex disability and trauma
  • Own car and driver's license
  • Strong interpersonal skills and high levels of resilience
  • Understanding of the NDIS
  • Working knowledge of Microsoft office suite and employee rostering systems
