Key Role in Supporting Complex Care Participants

Care participants living in our SIL Homes require high-quality support. To ensure we meet their unique needs, we are seeking a Service Delivery Manager to oversee end-to-end case management and conduct regular SIL Home visits.

The ideal candidate will have experience in managing diverse teams and demonstrated leadership skills in delivering service excellence in the disability sector. They will be responsible for developing human resource capability and ensuring safe delivery of NDIS client services through implementation of care models and frameworks.

Job Responsibilities:

  • Support a small team of Support Team Leaders to provide care and attendant services to complex care participants.
  • Oversee end-to-end case management relating to each participant living within the SIL portfolio.
  • Conduct regular SIL Home visits and audits to ensure safe systems of work in all SIL houses.
  • Be available for external stakeholders, families, and care teams to ensure continuity of quality services.
  • Coordinate documentation annually to ensure compliant services across the SIL client portfolio.
  • Identify areas of improvement and embed change initiatives.

Required Skills and Qualifications:

  • Tertiary qualification in a related field or significant skills and experience.
  • Demonstrated experience in aged care/disability/community care.
  • Comprehensive knowledge and understanding of NDIS, disability sector, current reforms, relevant legislation, funding, and policy protocols.
  • Experience in managing diverse teams and demonstrated leadership skills in managing an operational team in the disability sector.
  • Highly developed negotiation, problem-solving, and analytical skills, including strong interpersonal, verbal, and written communication skills.
