About the Role

We are seeking an experienced professional to join our team as a Disability Services Coordinator. This dynamic role will act as the conduit between our NDIS participants and support crew, managing the day-to-day coordination of support delivery.

Key Responsibilities
  • Roster Management: Coordinate the deployment of support workers to ensure seamless delivery of services.
  • Ongoing Liaison and Communication: Build strong relationships with clients to understand their needs and goals, and facilitate effective communication with support workers.
  • Intake and Risk Assessment: Conduct thorough assessments of new participants to identify their needs and develop personalized support plans.
  • Utilization of Carelink+: Manage the deployment of staff, approve completed work, and ensure accurate billing.
  • Matching of Support Workers: Pair support workers with clients based on their needs and preferences.
  • Recruitment and Selection: Identify and recruit high-quality support staff to join our team.
  • Model, Mentor, and Monitor: Provide guidance and support to staff to ensure they work within the NDIS Code of Conduct framework.
  • Ongoing Monitoring and Nurturing: Foster a team culture that prioritizes customer service and excellence.
Requirements
  • Proven experience working in the Disability or Health Sector.
  • Demonstrated experience in rostering and coordinating disability services.
  • Knowledge of NDIS Framework and Community Services Funding Models.
  • Working knowledge of best practice rostering, workforce optimization, and system utilization.
  • Experience managing workforce issues, including supervision, coaching, and mentoring.
  • Experience managing complex disability presentations and complexities.
  • Experience in incident management and reporting.
  • Experience delivering outcomes and working with KPIs.
  • A current Driver's Licence (non-negotiable).
  • Ability to work onsite 5 days per week.
