Job Title

A Community Care Leader position is now available at our organisation. This role involves leading a multidisciplinary team to deliver high-quality care to Aboriginal Home Health customers.

About the Role

This community of health and care professionals empowers their Mob to understand wellbeing needs and navigate healthcare systems, networks, and relationships.

Responsibilities
  • Lead a team of Care Team Leaders and inspire the broader neighbourhood team, Practice Leads, and multidisciplinary teams (MDTs) to achieve customer-centric outcomes.
  • C Coordinate day-to-day operations of the neighbourhood to ensure safe, high-quality care delivery.
  • Develop partnerships with external health partners and agencies to strengthen service integration and delivery.
  • Foster a high-performing team with the right values, behaviours, and competencies.
Requirements
  • Certificate IV or Diploma in a relevant field, Business qualification, or equivalent experience.
  • Operational and frontline experience in the care industry.
  • Experience leading geographically dispersed teams.
  • Excellent verbal and written communication skills for engaging with a frontline workforce across multiple channels.
  • Proficiency in Microsoft Office applications.
  • Knowledge of regulatory requirements, including funding types and aged care quality standards.
  • Ability to build partnerships and drive outcomes through others.
