Overview

Neighbourhood Leader - Identified Position at Australian Unity. Lead and inspire a multidisciplinary team to deliver high-quality, person-centred care to Aboriginal Home Health customers while shaping the Service Delivery function. The Aboriginal Home Health Neighbourhoods are a community of connected health and care professionals who empower their Mob to understand wellbeing needs and navigate healthcare systems, networks, and relationships. They deliver quality, safe care enabling customers to live and age well at home or in a place of their choosing.

Responsibilities
  • Lead a team of Care Team Leaders and inspire the broader neighbourhood team, Practice Leads, and multidisciplinary teams (MDTs) to embrace new ways of working with a focus on customer outcomes.
  • Coordinate day-to-day operations of the neighbourhood to deliver safe, high-quality care.
  • Work with the broader ecosystem to strengthen the integration and delivery of services, as well as working relationships with external health partners and agencies.
  • Develop a high-performing team with the right values, behaviours, and competencies.
Qualifications and Experience
  • Certificate IV or Diploma in a relevant field, Business qualification (or equivalent experience).
  • Operational and frontline experience in the care industry.
  • Experience leading geographically dispersed teams.
  • Excellent verbal and written communication skills with the ability to engage with a frontline workforce across multiple channels.
  • Proficiency with Microsoft Office applications.
  • Working knowledge of regulatory requirements, including funding types and aged care quality standards.
  • Ability to build partnerships and achieve outcomes through others.
  • Experience working with Aboriginal Medical Services (AMS) is preferred.
