Community Care Leader
As a Community Care Leader, you will lead and inspire a multidisciplinary team to deliver high-quality care to customers while shaping service delivery. You will empower your team to understand wellbeing needs and navigate healthcare systems, networks, and relationships.
Key Responsibilities- Lead a team of care professionals and inspire the broader team to embrace new ways of working with a focus on customer outcomes.
- Coordinate day-to-day operations of the community care service to deliver safe, high-quality care.
- Work with external health partners and agencies to strengthen integration and delivery of services.
- Develop a high-performing team with the right values, behaviours, and competencies.
- Certificate IV or Diploma in a relevant field, business qualification (or equivalent experience).
- Operational and frontline experience in the care industry.
- Experience leading geographically dispersed teams.
- Excellent verbal and written communication skills with the ability to engage with a frontline workforce across multiple channels.
- Proficiency with Microsoft Office applications.
- Working knowledge of regulatory requirements.
- Ability to build partnerships and achieve outcomes through others.
- Recognition of cultural responsibilities and access to cultural leave.
