Compassionate Care Coordinator

The Sidney Health Center is seeking a compassionate and skilled Care Management RN or LPN to join our I CARE team. This dynamic individual will work collaboratively with primary care clinicians and healthcare teams to ensure patients receive comprehensive and coordinated care.
Key Responsibilities:- Develop, implement, and monitor personalized care plans for clients ensuring all services are aligned with their health and wellness goals
- Serve as the primary point of contact for clients and their families, providing regular updates on care plans and addressing concerns
- Collaborate with healthcare providers, social workers, etc., to ensure clients receive comprehensive and coordinated care
