Compassionate Care Coordinator Role

This is a vital position within our HomeCare team, where you will oversee client portfolios, develop and manage care plans, and create services tailored to individual needs.

Main Responsibilities:
  • Client Management: Oversee client portfolios, develop and manage care plans that cater to their unique requirements
  • Service Coordination: Design and deliver services that meet the specific needs of clients
  • Administrative Tasks: Manage budgets, monitor financial transactions, make changes in care arrangements, and undertake marketing efforts
  • Staff Support: Collaborate with staff to deliver exceptional services to clients
Key Requirements:
  • Demonstrated experience in home care packaging (HCP), CHSP, DVA, case management, assessment, care planning, and service coordination
  • Hold a current CPR Certificate
  • Possess a solid understanding of client-centred, community-based care service delivery and associated quality standards
  • Have experience working with and adhering to budgets
  • Ability to liaise with and promote services to external providers
  • Capacity to build and maintain interpersonal relationships with staff
  • Hold a current driver's license with own fully insured vehicle
  • Proof of Covid-19 vaccination status
